Before Your First Visit.
You will receive an appointment reminder e-mail that contains a link to our New Patient Intake Form. Please complete this online form and submit it prior to your first appointment.
If you have had any radiologic studies done in the past 5 years (MRI, x-ray, CAT scan, etc) please bring a copy of the images on CD. If you do not have a copy of the images on CD, you must get a copy these images on CD prior to your appointment and bring the disc with you. Please DO NOT have the imaging center or your physician’s office mail us the CD – this can take up to a month and will delay your appointment.
Print and fill out the “Medical Records Release” and “Consent and Agreement to Pay” forms found below and bring these with you to your first visit.
What to bring to your first visit.
Please bring a picture ID, insurance cards and copay. You will also need to bring your completed Medical Records Release and Consent and Agreement to Pay forms and any imaging on CD. Failure to do so may result in a rescheduling of your appointment.
What to expect at your first visit.
At your first visit, our physicians will gather detailed information about your current symptoms, go over all medications and treatments tried in the past, review any imaging related to your current problem(s), and complete a thorough exam focused on determining the source of your pain.
We will provide detailed information about your findings in easy-to-understand terms to help you understand the cause(s) of your pain. We will then work with you to develop a comprehensive treatment plan, which may include additional imaging, optimization of your medication regimen, physical therapy or exercise programs to maximize improvement in function, and procedures designed to address the pain at its source.
